Here we have provided some forms and documents you may need if you are interested in participating in St. Mary’s Home and School Association:
- 2025/2026 Project Funding Request Form
- Complete this form for the funding of items not already budgeted for by the Home and School Association, including an itemized cost estimate. Cost estimates from at least two sources should be included (if possible). Be sure to include tax, shipping & handling costs, etc. if applicable.
- 2025/2026 Payment/Check Request Form
- For payment of previously approved funding requests or items already planned for in the current year's Home and School Budget, please complete this form.
